Liberty Health is seeking a Licensed Practical Nurse to provide compassionate, person-centered care within our behavioral health and human services programs. The LPN will deliver direct nursing care, administer medications, monitor vital signs, and support interdisciplinary treatment plans for individuals with behavioral health and developmental needs. Responsibilities include patient assessments, documentation, health education, and collaboration with RNs, physicians, and therapists. Responsibilities

  • Provide direct patient care in behavioral health and human services settings
  • Administer medications and treatments per provider orders
  • Monitor, document, and report vital signs and patient status changes
  • Support and implement interdisciplinary care and treatment plans
  • Perform basic patient assessments and contribute to care planning
  • Maintain accurate, timely clinical documentation and reports
  • Educate patients and families on health, medications, and self-care
  • Collaborate with RNs, physicians, and therapists to coordinate care
  • Adhere to clinical protocols, safety standards, and regulatory requirements
  • Support quality improvement and evidence-based practice initiatives
