Social Worker Key Responsibilities:- Provide emotional and psychosocial support to residents and families
- Assist with care planning, discharge planning, and resident advocacy
- Ensure compliance with federal, state, and facility regulations
- Collaborate with healthcare teams to enhance resident quality of life
- Bachelor's degree in Social Work (Master’s preferred)
- LSW license in Ohio
- Experience in long-term care or healthcare setting preferred
- Strong communication and problem-solving skills
- Knowledge of state and federal regulations for nursing homes
