Position Summary

The Home Health Scheduler is a highly organized and detail‑oriented colleague working in a fast‑paced, dynamic environment. The Scheduler coordinates patient and clinician schedules with accuracy to ensure seamless communication between patients and the clinical providers. The Scheduler utilizes excellent verbal and written communication skills, strong problem‑solving abilities and sound judgment to effectively facilitate daily operations of the home health agency.

The Home Health Scheduler is responsible for the scheduling of home health disciplines including skilled nursing, physical therapy, occupational therapy, speech language pathology, medical social services and home health aides. Schedules are coordinated with multiple providers to achieve efficient utilization of service and compliance with regulations and medical orders.

Department Summary

A Home Health Agency is a Medicare Certified organization that provides skilled medical and therapeutic services to patients in their place of residence. A Home Health Agency bridges the gap between inpatient care and independence at home, ensuring patients recover or manage chronic conditions effectively. Home Health services are provided on an intermittent basis by nurses, therapists, social service providers and home health aides.

Clinicians are required to drive their private vehicles to patient’s residences within the Agency’s service area which includes all or part of 9 counties. Aultman Health Care In Your Home is a department of Aultman Hospital.

Skills
  • Proficiency with Microsoft Office products including Outlook and Word.
  • Develops and maintains proficiency with the electronic medical record (EMR).
  • Ability to quickly learn home health regulations and apply those regulations to the scheduling process.
  • Works independently and with attention to detail.
  • Excellent interpersonal, oral, written and telephone communications skills.

Department Skills
  • Familiarity with the geographic service area.
  • Utilizes tools within the EMR and mileage calculators to maximize efficiency of clinicians.
Working Conditions
  • The majority of work occurs during agency hours of operation with some flexibility to work before or after hours of operation and overtime to meet the needs of the agency.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.
  • 100% on‑site work.
  • Office environment.
  • Coordination of break times with other colleagues required.
  • The majority of work occurs between 8:00 a.m. and 4:30 p.m. Monday through Friday.
  • Flexibility to work outside these hours to meet agency needs required.
Hazardous Exposure Category

Category I – Includes tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job‑related tasks that involve an inherent potential for spills or splashes of, or mucous membrane or skin contact with blood, body fluids or tissues are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
