Speech Language Pathologist

SUMMARY

The Speech Language Pathologist (SLP) provides comprehensive skilled care as ordered by the attending physician within a specified geographical territory. The SLP educates patients and their family members, ensures patient safety, completes accurate documentation, and remains compliant with all legal rules and regulations. The Company adopts the speech‑language pathologist scope of practice as outlined in Chapter 4753 ORC for its licensed SLP personnel.

• Provide skilled speech therapy as ordered by the attending physician.
• Evaluate the patient’s level of functioning by applying diagnostic and prognostic procedures.
• Develop an individualized speech‑therapy plan of care and adjust as needs change.
• Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
• Inform health‑care staff of speech‑therapy phases for collaboration.
• Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication.
• Guide the patient in using therapeutic, creative, and self‑care activities to improve functioning.
• Promote continuity of care with appropriate admissions, transfers, and discharges.
• Counsel and instruct patient and family about the disease, injury, disability, and how to manage throughout all phases of speech therapy.
• Incorporate patient and family in the development of the plan of care.
• Instruct patient and family on proper use of procedures and equipment.
• Teach patients preventative measures to help decrease re‑hospitalizations and make good decisions.
• Listen to patient and family members.
• Complete all clinical documentation per agency protocol and Medicare/Federal guidelines.
• Document all aspects of treatment, assessments, and patient education.
• Complete chart audits at the agency location as requested.
• Communicate with scheduler about any necessary changes.
• Participate in on‑call requirements and case conferences.
• Attend in‑service trainings and mandatory agency meetings.
• Participate in education programs and in‑services mandated by state law.
• Participate in orientation/training of new employees.
• Maintain a professional appearance as a representative of the company.
• Understand and practice agency policies and procedures.

• A current Speech‑Language Pathologist license in the state of employment.
• Minimum of one year of experience as an SLP (preferred).
• Home‑care experience is desired.
• Effective communication and the ability to create positive impressions.
• Knowledge of speech‑therapy best practices.
• Ability to make appropriate therapy judgments.
• Ability to identify a situation and handle it with the best solution.
• Detail‑oriented and observant.
• Disciplined work ethic with the ability to prioritize and be timely.
• Valid driver’s license and auto insurance with the applicant named as a listed driver.

Must be able to meet all physical position requirements. Evidence of a current negative tuberculin skin test or chest x‑ray is required at orientation; thereafter, an annual test is required unless previous documentation exists. Employees in this position may be at risk for exposure to blood borne pathogens. Physical demands include reading, speaking, writing, computer use, sitting, hearing, vision, walking, bending, standing, driving, lifting up to 50lbs, stooping, reaching, and occasional climbing.
