We are seeking a Patient Care & Admissions Coordinator who is organized, compassionate, dependable, and ready to thrive in a fast-paced healthcare environment.
This position plays an essential role in supporting patient admissions, coordinating care and transportation, maintaining accurate clinical documentation, communicating with families and healthcare providers, and assisting the clinical team with day-to-day patient needs.
Position SummaryThe Patient Care & Admissions Coordinator is responsible for supporting the admission process, ensuring accurate and timely documentation, coordinating patient transportation and appointments, and facilitating communication between patients, families, providers, pharmacies, hospitals, and other facilities.
The ideal candidate demonstrates professionalism, empathy, strong organizational skills, attention to detail, and the ability to remain calm and positive in a challenging healthcare environment.
Key Responsibilities- Prepare and organize patient admission documents and assist patients with completing admission paperwork.
- Obtain and verify copies of identification and insurance cards.
- Communicate patient updates, concerns, and relevant information with pharmacies, hospitals, families, facilities, physicians, therapists, and other appropriate providers.
- Coordinate transportation to and from groups, medical appointments, follow-up visits, and other approved activities.
- Assist with transporting patients when necessary and appropriate.
- Track patient injection due dates and communicate upcoming needs to the appropriate clinical staff.
- Help patients connect with additional community and healthcare resources when needed.
- Audit patient charts for completeness and accuracy.
- Collect required signatures and update admission orders as necessary.
- Send psychiatric orders, psychiatric progress notes, and other appropriate documentation to primary care providers, nursing facilities, or other authorized providers after receipt.
- Assist with patient care and provide support to the clinical team as needed.
- Communicate patient concerns promptly to physicians, therapists, and other members of the treatment team.
- Obtain updated medication lists from hospitals, pharmacies, residential facilities, and other healthcare providers.
- Assist physicians and providers with coordinating follow-up appointments and visits.
- Assist therapists with master treatment plans, treatment plan reviews, and discharge summaries.
- Provide coverage for therapist breaks by monitoring patients and assisting with lunch and beverages.
- Maintain accurate, timely, and confidential patient records.
- Perform additional duties and responsibilities as assigned.
- High school diploma or equivalent required.
- Degree from an accredited college or university in Social Work, Recreational Therapy, Behavioral Health, or another appropriate therapeutic field is preferred.
- A combination of relevant education and work experience may be considered.
Previous experience in a psychiatric or behavioral healthcare setting is preferred.
Experience working directly with individuals experiencing substance use disorders, dual diagnosis, mental health conditions, or other behavioral health needs is highly desirable.
License & Transportation Requirements- Must possess a valid driver's license.
- Must maintain a favorable Motor Vehicle Record (MVR).
