LPN ResponsibilitiesAdminister medications and treatments as ordered by the physician in accordance with Ohio nursing regulations
Monitor residents’ health status, including taking and documenting vital signs
Perform skilled nursing procedures such as wound care, catheter care, tube feedings, injections, and blood glucose monitoring
Assess changes in resident condition and promptly report findings to the supervising RN or physician
Contribute to the development and implementation of resident care plans under RN supervision
Supervise and delegate tasks to STNAs/CNAs as appropriate
Provide direct patient care and assist with activities of daily living when needed
Ensure accurate, timely documentation in accordance with facility and state guidelines
Communicate effectively with residents, families, interdisciplinary teams, and physicians
Respond appropriately to emergencies and follow facility protocols
Maintain infection control standards and ensure safe medication administration practices
Uphold resident rights, dignity, confidentiality, and safety at all times
Participate in required in-service training and staff meetings
Perform other duties within LPN scope of practice as assigned
QualificationsActive LPN license in the state of Ohio
Current BLS certification
Physically able to lift 50 lbs and assist residents with mobility
Strong communication skills; ability to read, write, and speak English
Previous experience in assisted living, long-term care, or skilled nursing preferred
Experience with memory care residents is a plus
