As an RN Case Manager, you'll travel throughout the Canton area, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals – wherever they feel most comfortable.
Required Qualifications- Active RN License
- Valid Driver's License
- 1+ years hospice experience
- BLS Certification
- Hospice and Palliative Medicine background
