What You'll DoManage your own patient caseload in Assisted Living communities and homes across Northern Canton and Wooster.
Assess patients and create care plans that change as their needs change.
Perform skilled nursing care: wound management, medication oversight, and chronic disease support.
Document your work in our Electronic Medical Record (EMR) system on time and accurately.
Work with doctors, facility staff, therapists, and families to coordinate patient care.
Teach patients and families about their health and care plan.
Communicate professionally and compassionately with patients and team members.
Follow all clinical protocols and regulatory requirements.
What You Need Active Ohio Registered Nurse (RN) license in good standing.
At least 6 months of nursing experience (case management, assisted living, or home care preferred).Ability to assess patients and make nursing decisions on your own.
Experience with wound care and managing chronic diseases.
Know how to use Electronic Medical Record (EMR) systems and document at the point of care.
Valid driver's license and reliable transportation.
Strong communication skills and a service-focused mindset.
Nice-to-Have Skills Experience in Assisted Living or community healthcare.
IV therapy or infusion experience.
Wound Vac (VAC) or central line care experience.
Previous case management experience.
What We Offer Flexible Scheduling You control your daily schedule, caseload size, and how much you work each week. No micromanagement.
Transparent Pay We use a point-based pay model. You earn $48-$52 per point. This is approximately $300 per day and $1,500 per week. Most RNs earn around $70K annually with no income cap. RNs who do more than 30 points per week get a 10% bonus on extra points.
Manageable Territory Your service areas in Northern Canton and Wooster are tight and close together. You spend less time driving and more time with patients.
