Hospice On Call RN

Overview

Cherish-Hospice is seeking an On Call Registered Nurse (RN) in the Canton Area. Cherish-Hospice emphasizes how we treat employees, compliance, and transitioning patients from focusing on curing to providing care. Cherish-Hospice is built on three pillars of values: compassion for the patient, guidance every step of the way, and ensuring the patient’s quality of life with physical and emotional comfort.

• Covering incoming calls and making necessary patient visits and admissions
• Assessing clients keys: physical and mental wellness, needs, preferences and abilities, and using these to develop tailored care plans
• Collaborating with clients, family and friend support networks and health care professionals to implement care plans
• Listening to clients’ concerns and providing counseling or intervention as required
• Recording clients’ progress including charting referrals, home visits and other notable interactions
• Travel as necessary to service patients in their residence of care (home, assisted living facilities, long-term care facilities, and hospitals)

• RN License (Required)
• Driver’s License (Required)
• BLS Certification (Preferred)
• Hospice or Home Health: 1 year (Preferred)
• Hospice and Palliative Medicine (Preferred)
