Position Summary

The primary purposes of the Licensed Practice Nurse (LPN) are to provide direct patient care, patient and family teaching, and work cooperatively within the interdisciplinary health care team in maintaining standards of nursing care under the direction of the Registered Nurse (RN) and within the framework of the Ohio Nurse Practice Act.

Department Summary

Woodlawn Rehab is a comprehensive medical rehabilitation unit. The nurse collaborates with therapy (PT, OT, and ST) to provide patients with rehabilitative care.

Qualifications
  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.
  • IV Certification, preferred.

Responsibilities & Expectations
  • Demonstrates proficiency in basic nursing service, knowledge, and skills.
  • Demonstrates knowledge of and applies evidence-based practice.
  • Collects comprehensive data pertinent to the patient’s health or the situation.
  • Identifies and reports changes in patients’ status to Registered Nurse for evaluation.
  • Documents nursing activities accurately and in a timely manner.
  • Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment under the direction of the Registered Nurse.
  • Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients’ responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse.
  • Collects specimens as ordered.
  • Interacts with and contributes to the professional development of peers, colleagues, and students.
  • Collaborates with patient, family, and others in the conduct of nursing practice.
  • LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction.
  • Attains knowledge and competency that reflects current nursing practice and applicable policies.
  • Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules and regulations, during nursing practice.
  • Exhibits High Reliability Organization (HRO) tools and tones.
  • Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.
Working Conditions
  • Meal and break times are coordinated with other staff members to maintain adequate coverage.
  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.
  • Work weekends and holidays as assigned or as applicable based on department needs.
  • Cross training depending upon department needs.
