About This Role

The RN in the Transitional Care and Discharge Unit at Cleveland Clinic’s Mercy Hospital will provide exceptional nursing care to patients transitioning from acute care to home or another care setting. This role is ideal for dedicated caregivers who are passionate about enhancing patient experiences and ensuring safe discharges.

• Provide direct nursing care.
• Implement and monitor patient care plans.
• Monitor, record, and communicate patients’ conditions.
• Administer prescribed medications and treatments.
• Note and carry out physician and nursing orders.
• Assess and coordinate patients’ discharge needs.
• Educate patients and their families.

Education:

• Graduate from an accredited school of professional nursing (ADN or BSN program) required
• Bachelor’s of Science in Nursing (BSN) preferred

Experience:

• One year of current acute care RN experience in a hospital setting OR licensed military RN experience required

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross required
