Role DescriptionThis is a part‑time, Registered Nurse role based in Canton, OH, providing direct nursing care to clients in their homes and community settings. The Registered Nurse will conduct comprehensive assessments, develop and update individualized care plans, administer medications and treatments, and monitor clients’ responses to care.
Daily responsibilities include coordinating with physicians and other care team members, educating clients and families on disease management and home care procedures, and documenting all care accurately in accordance with regulatory and agency standards. The nurse will also supervise and support home health aides as appropriate, participate in case conferences, and respond to changes in client conditions to ensure timely interventions.
This role involves travel within the local area to client residences, adherence to infection control practices, and a commitment to patient‑centered, culturally sensitive care.
- Current, unrestricted Registered Nurse (RN) license in the state of Ohio.
- Prior experience in home health, acute care, long‑term care, or community nursing preferred.
- Strong clinical assessment, critical thinking, and care planning abilities.
- Effective communication and interpersonal skills for collaborating with clients, families, and interdisciplinary teams.
- Comfort with electronic health records and basic digital documentation tools.
- Valid driver’s license, reliable transportation, and ability to travel locally to client homes.
- Ability to work independently, manage time effectively, and prioritize multiple client needs.
- Current CPR/BLS certification and knowledge of infection prevention and safety protocols.
- Willingness to provide care to clients of diverse backgrounds and needs in a respectful, non‑discriminatory manner.
- Associate or Bachelor’s degree in Nursing from an accredited program.
