Join a dedicated healthcare team as an Afternoon Shift LPN/RN, providing exceptional, personalized nursing care to residents in a rehabilitation and health center setting.

• Supervising and coordinating the work of nursing assistants (CNAs).
• Delivering individualized care based on each resident’s plan of care.
• Assisting with admissions, transfers, and discharges.
• Administering medications and treatments as prescribed.
• Explaining procedures to residents to ensure cooperation and reduce anxiety.
• Monitoring vital signs and observing patient progress.
• Maintaining accurate medical records of nursing observations and actions.
• Documenting care needs to ensure continuity of care.
• Monitoring emotional well‑being and preparing residents for ongoing care.
• Performing additional duties to ensure quality resident care.

• Active, unencumbered Ohio LPN or RN licensure.
• Excellent communication and interpersonal skills.
• Highly organized, detail-oriented, and capable of working independently or within a team.
