Summary

The RN Home Visitor plays a key role in delivering case management and support services within the Nurse Family Partnership program, focusing on pregnant and parenting women and their children.

Responsibilities include conducting home visits, collaborating with community agencies, and ensuring compliance with all regulatory standards, policies, and protocols. The role emphasizes maintaining high standards in clinical practice, professional development, teamwork, and research.

Qualifications include a valid RN license, BSN within five years, BLS certification, and at least two years of recent pediatric or related experience with children birth through five. Experience in adolescent psychology, OB, home care, or community nursing is preferred.

Skills required include strong communication abilities, especially with adolescents, and a valid driver’s license with insured transportation. The role is onsite and involves providing comprehensive physical, psychological, social, educational, and safety care tailored to patients' needs.

Other duties as assigned support the program's objectives.
