Licensed Practical Nurse​/Licensed Vocational Nurse

Responsibilities

• Provide direct nursing care to individuals in home and community-based settings
• Administer medications and treatments according to physician orders and regulations
• Monitor and document vital signs, health status, and responses to treatment
• Assist with development and implementation of individualized care plans
• Collaborate with interdisciplinary team members to coordinate services
• Educate individuals and families on health conditions, medications, and self-care
• Maintain accurate, timely clinical documentation in electronic health records
• Support individuals with behavioral and cognitive needs using de-escalation and therapeutic approaches
• Follow infection control, safety, and regulatory standards at all times
• Participate in ongoing training and professional development offered by Sevita
