RN - Nurse Family Partnership Program

Role Overview

The RN Home Visitor is responsible for providing case management and support services to pregnant and parenting women and their children under the Nurse-Family Partnership program.

• Conduct home visits with assigned clients, adhering to research protocols and program guidelines.
• Collaborate with community agencies to coordinate care and services.
• Ensure compliance with the Ohio Nurse Practice Act, regulatory standards, policies, procedures, and protocols of relevant departments and funding sources.
• Address the physical, psychological, social, educational, and safety needs of patients across all ages.
• Perform other duties as required to support program goals.

• Registered Nurse licensure and BLS certification are mandatory.
• BSN degree required within five years.
• Minimum of two years recent experience in pediatrics or related fields with children birth through five years.
• Strong communication skills, especially with adolescents.
• Valid driver's license and insured vehicle required.
• Preferred experience in adolescent psychology, OB, home care, and community nursing.

This is a full-time, onsite position with a focus on professionalism, teamwork, and quality care delivery.
