Position: Community Case Manager - Flexible, Impactful Support

Pathway Caring for Children is a trusted nonprofit organization with more than 50 years of service helping youth and families thrive through community-based mental health services and family support. We're currently hiring a Community Case Manager to provide hands-on support to children and families in their homes, schools, and community settings.

Job Snapshot
  • Position: Community Case Manager (Children & Families)

  • Schedule: Weekdays (Monday–Friday) with some afternoon/evening hours

  • Job Type: Full-Time

  • Work Setting: Community-based (homes, schools, community locations)

  • Travel Required: Yes - must have reliable transportation and valid insurance

  • Training Provided: Yes - paid training, ongoing mentorship, and support

What You'll Do

As a Community Case Manager, you'll support youth and families through individualized, goal-focused services. You will work closely with licensed therapists and other team members to help clients strengthen coping skills, improve stability, and succeed at home, school, and in the community.

Key responsibilities include:
  • Provide individualized support services to children and teens in home, school, and community settings

  • Assist clients in working toward treatment goals through structured skill-building interventions

  • Facilitate engaging group sessions on topics such as:

    • social skills

    • emotion regulation

    • conflict resolution

    • coping strategies

  • Advocate for families and connect them with community resources and supports

  • Maintain accurate, timely documentation to support quality care and compliance

  • Communicate regularly with supervisors and clinical team members to coordinate services

Schedule & Work Environment

This is a community-based role serving clients in real-world environments. Most work is completed during weekdays, with some afternoon/evening availability required to meet client needs.

You may work in:

  • homes

  • schools

  • community settings

  • office/meeting spaces

What We're Looking For

We're seeking motivated, compassionate individuals who want meaningful work and enjoy supporting children and families in the community.

Minimum qualifications:
  • Must be at least 21 years old

  • High school diploma or GED required

  • Valid driver's license, reliable transportation, and appropriate auto insurance

  • Strong organizational and time-management skills

  • Ability to work independently while staying connected with a team

  • Must pass required background clearances per program guidelines

Preferred qualifications:
  • Experience working with children, teens, or families

  • Experience in mental health, social services, education, or community support

  • Knowledge of community resources and family systems
