Join a Team That Makes a Difference For over 40 years, Caprock Home Health has provided compassionate, patient-centered care across Texas. We are seeking a dedicated Registered Nurseto join our team and support clients in maintaining independence and quality of life at home.
Key Responsibilities- Conduct pre-screening visits and assess clients for level of care and service needs
- Complete admissions, required program documentation, and state forms in a timely manner
- Develop, implement, and monitor individualized service plans based on client needs and program guidelines
- Document diagnoses, medications, supplies, therapies, and service needs; coordinate follow-up to ensure timely delivery
- Supervise and monitor personal care attendants and delegated nursing tasks to ensure safe, quality care
- Conduct routine client visits to evaluate service plans, client needs, and attendant performance
- Coordinate with clients, families, case managers, and agency staff regarding services and plan changes
- Ensure compliance with waiver program requirements, infection control standards, and agency policies
- Complete accurate and timely clinical documentation and required reports
- Participate in on-call rotation and provide skilled visits as needed to support client health and safety
- Support continuity of care when staffing coverage is needed
- Collaborate with leadership to maintain quality outcomes and regulatory compliance
- Graduate of an accredited school of nursing
- Current Texas Registered Nurse license (or compact multistate license)
- IDRC Assessment training/certification required (or ability to obtain upon hire)
- One year of nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Knowledge of home health and waiver program regulations preferred
- Strong communication and documentation skills
- Ability to interpret program regulations and work independently
- English proficiency required;
Spanish speaking is a plus - Valid Texas drivers license and reliable transportation
- Clear criminal background check
- Ability to travel independently to client homes
- Ability to lift and carry medical supplies and assist with patient mobility as needed
- Ability to perform physical tasks related to patient assessments and nursing care, including transfers and use of adaptive equipment
