Nurse Practitioner, Public Health Nurse
An innovative mission-driven healthcare organization is seeking an experienced Family Nurse Practitioner to join its primary care team on Cape Cod,
Massachusetts

This community-focused organization provides medical, behavioral health, dental, and supportive services to more than 34,000 patients across Boston, the South Shore, and Cape Cod.
- In this role, you will deliver comprehensive, patient-centered care across the lifespan, with a focus on prevention, chronic disease management, and acute care.
- This is a full-time opportunity and may include evening hours and a rotating Saturday schedule based on clinic needs.
- Salary Range: $124,800 to $ annually

- Serve as the primary care provider for an assigned patient panel
- Deliver comprehensive care including preventive services, acute care, and chronic disease management
- Conduct routine wellness visits including full histories, physical exams, screening, and patient education
- Coordinate and manage care, including referrals to specialists, behavioral health, nutrition, and social services
- Contribute to evening, weekend, and holiday clinic coverage as needed
- Graduate of an accredited Family Nurse Practitioner program
- Current Massachusetts Nurse Practitioner license or eligible
- Board Certification as a Family Nurse Practitioner
- Current DEA, MCSR, and Basic Life Support (BLS) certification
- Minimum of 2 years of Nurse Practitioner experience
- Proficiency with computers and EMR systems (EPIC/OCHIN experience preferred)
- Experience in a community health center or similar setting strongly preferred
- Bilingual skills a plus (English/Portuguese/Spanish)
