Overview
Lee Health is seeking an experienced Diabetes Educator and Certified Diabetes Care and Education Specialist to help meet the needs of our growing diabetes population. This outpatient clinic position has a hybrid schedule of 3 days in office and 2 days at home, and requires travel to multiple locations.
Sign‑on bonus: $5,000.00 and relocation incentive: $7,500.00.
Responsibilities• Manage the care of a defined patient population.
• Develop expected patient outcomes, define appropriate resource utilization, and set customer satisfaction goals.
• Serve as a clinical expert providing education and consultation services to internal and external groups.
• Collaborate with case managers, social workers, physicians, and other caregivers to facilitate care across the continuum.
• Teach classes in the outpatient setting and contribute to new initiatives of Lee Diabetes Care.
- Associate degree in a health‑related profession.
- Minimum 2 years of professional practice experience in Diabetes Self‑Management Education; 1000 hours within the past 5 years preferred.
- Certified Diabetes Care and Education Specialist (CDCES) required; national registration as an RD (if dietitian) required.
- Current license as an RN in Florida or Florida Licensed Dietitian/Nutritionist.
- Excellent communication and interpersonal skills; flexibility required.
