Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.

The Diabetes Registered Nurse Educator (RN) is a professional nurse with specialized knowledge and skills who demonstrates leadership when providing, directing, and improving care for patients with diabetes mellitus, Type I, Type II, and gestational diabetes. Collaborates with other disciplines/departments to achieve desired patient outcomes. Uses a team approach to identify, analyze, and resolve patient care problems with measurable outcomes. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation.

Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Evaluates practice in an ongoing process, based on best evidence. Provides peers with informal constructive feedback for improvement. Fosters a safe and supportive environment conducive to the professional development of healthcare professionals. Contributes professional nursing perspective in discussions with the interdisciplinary team. Partners with others to effect change and produce optimal outcomes.

Supports colleagues through knowledge sharing to provide safe, quality nursing care. Shares educational findings, experiences, and ideas with peers. Questions clinical practices for the purpose of providing evidence-based care. Participates in activities and strategies to sustain an evidence-based-practice culture.

The Diabetes Nurse Educator is responsible and accountable for the development, promotion, and delivery of evidence based professional diabetes education to patients and families. They are responsible for the coordination and implementation of a process to develop diabetes education course materials and course instruction, including assessment and planning objective content, implementation of the plan and evaluation tools appropriate to the target audience of varying levels of education.

The educator must demonstrate strong planning and organizational skills to support customized education programs and events for veterans and nurses inpatient and outpatient care areas.

The Diabetes Nurse Educator must prepare clear, timely, relevant and current education reports as required by the VISN, VHA and TJC. The DNE collaborates across all service lines as needed to develop educational offering that improve organizational performance. The educator establishes a process to maintain easily accessible patient and staff education records as required for accreditation, regulatory agencies and risk management.

The Educator shall lead the Diabetes Resource Education Committee and participate in other committees aimed at providing diabetes education to veterans.

The Diabetes Nurse Educator assists veterans with latest technology such as insulin pumps and Continuous Glucose Monitoring Systems. They are also involved with ensuring safe hospital discharges for patients with diabetes. The Diabetes Nurse Educator functions as a resource to other nursing staff and meeting the criteria to maintain American Diabetes Association Accreditation. The Educator also provide care, education, and guidance to patients with Type I, Type II, and gestational diabetes.

They work with both individuals and groups. They educate patients and their family members about the causes, pathophysiology, progression and signs and symptoms of diabetes, and diabetes management.
