Job Summary
Heritage Park Care Center is seeking a compassionate and detail-oriented Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will provide direct, hands‑on nursing care to residents in a long‑term care and rehabilitation setting, ensuring their comfort, dignity, and safety at all times. You will administer medications and treatments as ordered, monitor and document vital signs and patient responses, assist with daily living activities, and promptly report changes in residents’ conditions to the supervising Registered Nurse or physician.
Working closely with an interdisciplinary care team, you will help develop and follow individualized care plans, coordinate with families, and support a resident‑centered approach that prioritizes respect, empathy, and clear communication.
Perform wound care, catheter care, and other technical nursing procedures within your scope of practice, maintaining strict adherence to infection control and safety protocols. Accurately complete electronic health records and documentation, ensure proper medication administration and storage, and help maintain compliance with state and federal regulations. Mentor nursing assistants, support their daily tasks, answer call lights in a timely manner, and contribute to a positive, team‑focused work environment.
WorkSchedule & Expectations
This position requires flexibility to work rotating shifts, including some evenings, nights, weekends, and holidays, to ensure 24/7 coverage for our residents. The ideal candidate is reliable, patient, and able to remain calm and professional in urgent or emotionally stressful situations. Strong communication and interpersonal skills are essential, as you will regularly interact with residents, families, and colleagues from multiple disciplines.
