Position: License Practical Nurse (LPN)

What You’ll Do

As an LPN Carbondale Rehabilitation and Healthcare Center:

  • Provide compassionate, direct nursing care to residents while following individualized care plans

  • Administer medications and treatments accurately and safely

  • Closely monitor residents’ conditions, promptly identify changes, and communicate effectively with RNs and physicians

  • Maintain thorough, timely, and accurate documentation

  • Guide and supervise Certified Nursing Assistants (CNAs) to ensure consistent, high-quality care

  • Support smooth admissions, transfers, and discharges

  • Build trusting relationships with residents, families, and the entire care team through clear and caring communication

  • Uphold the highest standards of infection control and resident safety

What You Bring
  • Active and unrestricted LPN license in the state of Pennsylvania

  • Passion for geriatric and rehabilitative care

  • Strong clinical skills, attention to detail, and a collaborative spirit

  • Commitment to dignity, respect, and person-centered care
