Aventura at Creekside is seeking an experienced, driven, and relationship-focused Admissions Director / Hospital Liaison to lead our admissions efforts and strengthen partnerships with hospitals, physicians, case managers, and community referral sources.

Key Responsibilities- Develop and maintain strong referral relationships with hospitals, physicians, case managers, and other healthcare partners.
- Serve as the primary liaison between referral sources and the facility.
- Coordinate and manage the admissions process from referral through admission.
- Evaluate referrals to ensure appropriate clinical and financial acceptance.
- Collaborate with the interdisciplinary team to ensure smooth resident transitions.
- Conduct regular hospital and community visits to generate new referral opportunities.
- Track admissions activity, referral trends, and census goals.
- Participate in marketing initiatives, networking events, and community outreach.
- Maintain compliance with all federal, state, and facility policies.
- Previous experience in healthcare admissions, business development, hospital liaison, or clinical marketing required.
- Skilled Nursing Facility (SNF), rehabilitation, long-term care, or hospital experience strongly preferred.
- Excellent communication, relationship-building, and negotiation skills.
- Strong organizational and time management abilities.
- Ability to multitask and work independently in a fast-paced environment.
- Proficiency with Microsoft Office and electronic medical record systems.
- Valid driver's license and reliable transportation for local travel.
