Role Description

The Wound Care Licensed Practical Nurse (LPN) is a full-time, on-site role based in Carencro, LA, responsible for providing direct wound care and nursing support to residents and patients. Day-to-day tasks include assessing wounds, performing and documenting wound treatments, monitoring healing progress, and following physician and advanced practitioner orders. The LPN collaborates with the interdisciplinary team to develop and implement care plans, provides education to patients, residents, and families on wound prevention and management, and ensures infection control practices are followed.

Additional responsibilities include administering medications within scope of practice, assisting with activities of daily living as needed, accurately documenting patient care in medical records, and participating in quality improvement and safety initiatives.

Qualifications
  • Active Licensed Practical Nurse (LPN) license in the state of Louisiana and a strong foundation in general Nursing skills.
  • Demonstrated Wound Care skills, including assessment, dressing selection, treatment implementation, and monitoring of healing.
  • Experience in Patient Care, with the ability to provide compassionate, person-centered support in a clinical or long-term care setting.
  • Background in Home Care or similar settings is beneficial, with skills in coordinating care and supporting patients’ ongoing recovery needs.
  • Additional qualifications:

    current BLS/CPR certification; strong communication and documentation skills; ability to work collaboratively with an interdisciplinary team; commitment to infection control, safety, and ethical practice; willingness to work rotating shifts, weekends, and holidays as needed; prior experience in long-term care or wound care certification preferred.
