• Provide physical therapy assistance under the direction and supervision of a licensed Physical Therapist
  
  PT)
• Perform therapeutic interventions designed to improve patient strength, mobility, and functional independence
• Assist patients with active and passive therapeutic exercises, muscle re-education, gait and functional training, transfers, prosthetic training, and activities of daily living
• Utilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage, as outlined in the plan of care
• Observe, document, and report patient treatment, response, and progress to the Physical Therapist, Clinical Manager, and Health Care Provider on a timely basis
• Promptly report significant changes in patient condition (physical or emotional), safety concerns, unusual occurrences, or adverse effects of medical devices
• Provide care and complete documentation in compliance with State, Federal, Medicare, Medi-Cal, and third-party payer regulations and standards

• Current license as a Physical Therapist Assistant in the state of California
• At least one year of work experience under the supervision of a licensed physical therapist
• Must have a vehicle, valid current driver’s license and automobile insurance
• Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner
• Home Health and Home Care Home Base documentation experience preferred but not required
