Overview

Licensed Vocational Nurse (LVN), Home Health - San Diego (North County). Full-time position. Associate's degree required. Hourly pay: $34.00 - $42.00 per hour. Coverage throughout North San Diego County, including Vista, Oceanside, Escondido, San Marcos, and surrounding communities.

San Diego Home Health is a trusted home health provider delivering compassionate, one-on-one care in the home. We serve patients across San Diego, Temecula, and Riverside Counties, with over 25 years of service.

• Provide direct patient home care visits, observe, perform and report preventative, treatment and rehabilitative nursing procedures while implementing the plan of care
• Observe and report on the home environment and make recommendations that enhance patient care, promote safety, and prevent adverse effects
• Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient outcomes
• Take verbal orders from Health Care Providers as necessary within the LVN scope of practice and document appropriately
• Inform Health Care Providers of changes in patient conditions per providers’ orders and document communications
• Provide care as ordered that is preventive, rehabilitative, therapeutic, comfort and pain interventions
• Provide instructions to patient/CGs and staff in a timely manner

You are experienced and ready to bring your skills and knowledge to patients who need it most. You excel in building meaningful relationships with patients and their families. You are committed to high-quality nursing care while working independently in a home setting.

• Graduation from an accredited nursing program, current state license as a Licensed Vocational/Practical Nurse, and CPR-certified
• At least one year of clinical practical nursing experience;
  Home Health experience preferred
• Must have a vehicle, a valid driver s license, and automobile insurance
• Ability to make home visits and provide skilled nursing assessments and care in the home
