Job Description Overview
This role supports a new Psychiatry practice and requires a high degree of flexibility. The schedule is Monday-Friday, 8:00 AM-5:00 PM
, with no weekends or holidays
. This is an onsite position located at North MOB
. Travel to other facilities will be required during the initial 3‑week training period
.
- Serve as a liaison between physicians, providers, and patients
- Provide referral resources and ensure patient needs are met outside of therapy
- Support patient registration and onboarding
- Deliver patient education and guidance
- Manage patient caseloads through in‑person, telephonic, and electronic communication
- Conduct outreach calls with patients/caregivers to encourage program participation
- Coach patients to help them meet personal and clinical goals
- Identify patients needing intensive case management
- Assist with scheduling provider visits, arranging transportation, and preparing visit agendas
- Educate patients on preventive monitoring and help caregivers understand response plans
- Help patients access community and government-based service agencies, including assisting with paperwork
- Document all activities and reconcile patient information to support clinical team productivity
- Collaborate with the multidisciplinary clinical care team
- Conduct patient interviews and assist with program evaluation
- Recommend improvements to program processes
- Background as an RN or LPN is helpful
- Degree in Social Work or prior mental health care experience
- Strong patient engagement and communication skills
- Ability to work independently with minimal oversight
- High integrity and professionalism
- Experience delivering excellent patient experiences
- Comfort working in a flexible, evolving practice environment
- High school diploma or equivalent required
- Bachelor's degree preferred
- Medical Assistant (MA)
- Licensed Practical Nurse (LPN)
- Bachelor of Social Work (BSW)
- Clinical Dietitian
All requiring current and appropriate accreditation/licensure
- Experience working with patients who have chronic health conditions and behavioral health needs
- Demonstrated ability to work effectively as part of a multidisciplinary clinical team
