As an LPN in home health, you will provide compassionate, patient-centered care in home settings and support positive outcomes for patients and families. • Location: Carmel, IN
  - Greater Indianapolis Area (Hamilton County)
• Hours per Week: 40
• Shift: Monday
  - Friday, 8:00 AM - 5:00 PM (Day Shift)
• Shift Duration: 8-hour days
• Contract Length: 13 weeks (travel LPN assignment)
• Mileage Reimbursement: $0.43 per mile
• Estimated Weekly Pay: $1460-$1700/week

• Active LPN license in Indiana or compact state (Licensed Practical Nurse)
• Minimum 1 year of home health experience required (homecare / home health nurse)
• Proficiency with Home Care Home Base (HCHB) software
• Strong patient relations, clinical communication, and documentation skills
• Ability to use Microsoft Teams and Outlook for interdisciplinary communication

• Provide high-quality home health nursing care to patients in their homes across Carmel and the Greater Indianapolis area
• Perform patient assessments, administer medications, and monitor patient progress during homecare visits
• Document care accurately using HCHB (Home Care Home Base) and communicate updates through Teams and Outlook
• Collaborate with interdisciplinary healthcare teams to coordinate care plans and ensure optimal patient outcomes
• Educate and support patients and family caregivers on care plans, medication administration, and self-care
