Hospice Registered Nurse RN Case Manager Full Time Carol Stream IL Journey Care Hospice

• Consistent Monday Friday schedule
• Only 1 weekend every 12 weeks significantly less than many competitors

• One weekend rotation every 12 weeks
• Territory Carol Stream IL

• annually based on experience

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments admissions and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
  
  Provide end of life support including family education and pronouncement
• Maintain timely accurate documentation
• Supervise Home Health Aides in compliance with regulations

• Graduate of an accredited RN program
• Active Illinois RN license
• Current CPR and First Aid certification
• Hospice or home health experience preferred
• Strong clinical judgment and communication skills
• Valid drivers license and proof of insurance
