This grant‑funded position facilitates group classes for caregivers, assists program participants in accessing community resources, and works with volunteers to help clients adjust to life in the U.S. The role also supports case managers and provides Medicaid‑funded case management services. Successful candidates demonstrate a proven ability to assist immigrants in achieving stability and overcoming barriers that hinder healthy integration.
Role & Responsibilities Group Services- Facilitate topic‑specific group sessions for caregivers, covering self‑care, health promotion, psychosocial education, and caregiving resources, to build support and community connection.
- Develop curriculum and pre/post‑tests for each topic.
- Collaborate with volunteers who provide transportation, childcare, materials, or additional facilitation for group sessions.
- Partner with the Services to Older Refugee (SOR) team to provide meaningful group services that include both caregiver and older adult participants.
- Meet with ethnic community leaders to assess needs and promote group programming.
- Collaborate and provide training for community partners, mutual aid associations, and churches.
- Coordinate with appropriate World Relief staff around service provision as necessary.
- Conduct plans of care, assess needs, strengths, barriers, and set goals; conduct monthly check‑ins to ensure goal achievement.
- Provide case management supportive services to individuals funded through Medicaid, performing all necessary assessments and follow‑up via IM+CANS.
- Exit clients within 3–9 months after all goals are completed; conduct assessments and close out plans of care.
- Assist other case specialists with case‑management tasks when capacity allows.
- Complete additional duties as assigned.
- Write detailed case notes for each client interaction; maintain records in Ritten for Medicaid and Dynamics for all other services.
- Retrieve signatures from participants and complete all necessary program paperwork.
- Write monthly stories and provide input for grant reports.
- Research topics and gather evidence‑based materials for group sessions.
- Attend all department, team, and staff meetings as scheduled.
- Participate in regular Medicaid meetings held off‑site to discuss cases and related mental health topics.
- Support seasonal or special events as needed.
- Mature and personal Christian faith.

- Desire to serve and empower the church to impact vulnerable communities.
- Able to affirm World Relief’s Core Beliefs, Statement of Faith, Christian Identity, and the National Association of Evangelicals’ “For the Health of the Nation” document.
- Valid driver’s license, reliable vehicle, and ability to pass an MVR check.
- Willingness to drive a 15‑passenger van.
- IM+CANS certified or willing to become certified.
- Regular travel required to client homes and service provider locations.
- Occasional evenings or weekends required.
- Ability to work with and supervise interns.
- Strong Microsoft Office skills.
- Four‑year university degree preferred.
- Self‑motivated and able to work independently, handling multiple tasks simultaneously.
- Ability to serve sensitively and effectively in cross‑cultural settings; cross‑cultural experience desired.
- Detail‑oriented with strong organizational skills.
