Role:
Nurse Practitioner providing hospice care in patients' homes.
Responsibilities- Conduct advanced assessments and manage patient symptoms.
- Develop personalized care plans in collaboration with an interdisciplinary team.
- Educate patients and families on disease progression, comfort care, and medication use.
- Travel to patient residences to deliver care and document clinical activities.
- Participate in team meetings and ensure compliance with hospice regulations and quality standards.
- Licensed Nurse Practitioner with relevant clinical experience.
- Strong assessment, communication, and patient education skills.
- Ability to work collaboratively with healthcare professionals and families.
- Comfort with travel and documentation requirements.
