Overview
Tanner Health is seeking a full-time Nurse Practitioner or Physician Assistant to join the Tanner Medical Group Bridge Clinic team, a transitional care facility designed to bridge the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP).
Responsibilities- Provide primary care services including medication management, chronic disease monitoring, coordination of care with specialists and patient education.
- Requires completion of an accredited Nurse Practitioner program, with a master’s degree in nursing OR master’s degree from an accredited Physician Assistant program
- Active GA APRN license OR GA Physician Assistant license or meet criteria for obtaining licensure.
- Must be board certified as a Physician Assistant.
- Prior experience in adult medicine is preferred.
- New graduates are welcome to apply.
- Full-time, Monday – Friday, 8am – 5pm.
- Position will be located and require shifts in Carrollton and Villa Rica Bridge Clinic locations.
