Lifecare Home Health - - Responsibilities:
Provide direct patient care to patients under the direction of the registered nurse in the patient's home;
Assist with safe transfers and ambulation per Hospice policy;
Report changes and document pertinent information and care rendered to ensure continuity of care;
Maintain infection control principles and provide a clean, safe, and comfortable environment;
Participate in on-call/after-hours responsibilities and weekend/holiday coverage as assigned
