Position: Licensed Practical Nurse (LPN) - Home Health (Days) (PRN)

Hospice Care

Assumes the responsibility for identifying patient family needs and for providing clinical care in accordance with physicians orders and plan of care within the policies and procedure of Hospice Care.

Education

Technical or Vocational school plus up to 2 years specialty training

Experience

One year of related experience. Requires a working knowledge of standard practices and procedures.

Licenses & Certifications

* Driver's License
* Georgia Licensed Practical Nurse or Multistate Nursing License
* Healthcare Provider (CPR) required within 30 days of hire.
* Vehicle Insurance

Qualifications

* A minimum of one (1) year experience as a LPN in an acute care setting preferred.
* BLS required within 30 days of hire.
* Dependable automobile
* Experience in Hospice preferred
* BLS required within 30 days of hire.

Area of Responsibilities

* Assesses and evaluates patients on every visit.
* Implements appropriate nursing interventions to meet patient needs and change in conditions.
* Initiates plans for patient safety, involving the patient/family and community resources.
* Provides for ongoing communication with appropriate staff members to make known any changes in patient needs, schedules, physician orders, special instruction and patient updates.
* Verifies the Plan of Care prior to each visit and provides care according to physician's orders and Agency policies and procedures.
* Adheres to all Infection Control guidelines in accordance with Agency policies and procedures.
* Anticipates and responds to the needs of all customers.
* Appropriately updates/revises Medication Profiles monthly or PRN, per Agency policy procedure.
* Attends all Case/Team Conferences regarding patients and provides input for care planning, as assigned.
* Customer Service
* Demonstrates flexibility and cooperation in working with others.
* Efficiency*Ensures that clinical documentation evidences coordination of services with physician, other disciplines and other appropriate staff members.
* Ensures that patient home charts are kept accurate and current.
* Established networks and seeks information from many sources.
* Follows up on problems so they are not repeated.
* Gives and accepts feedback appropriately.
* Listens effectively and communicates appropriately with others.
* Looks for and suggests ways to continually improve services.
* Make home visits on patients for the purpose of assessing, teaching, treatments, etc., as assigned.
* Provides nursing care requiring specialized skills, per current nursing practice and Agency policies and procedures.
* Provides patient care in accordance with the current Plan of Care.

* Writes Verbal Orders/Change Orders for any changes to the Plan of Care (including PRN visits) on the day of the identified change and/or visit.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.

Regularly works with some confidential data such as account, salaries, patient medical records, which if disclosed might have adverse internal or external effects.
