RN FIELD NURSE
Responsibilities- Administer skilled nursing care to patients/clients of all ages in their place of residence.
- Coordinate care with the interdisciplinary team, patients/clients and their families, and a referring agency.
- Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs; use OASIS or other appropriate assessments.
- Re‑evaluate patient/client nursing needs regularly and assess care outcomes.
- Develop, initiate, and revise the plan of care with patient/client, representative, and caregiver.
- Initiate appropriate preventive and rehabilitative nursing procedures; refer to other services as needed.
- Provide patient/client and caregiver education; plan discharge.
- Furnish specialized nursing services ordered by the physician.
- Counsel patient/client, caregiver, and family regarding nursing needs.
- Use infection control measures protecting staff and patients.
- Coordinate services, inform physician of condition changes, communicate with all involved physicians and other health practitioners; integrate orders.
- Monitor assigned cases for compliance with third‑party payor requirements.
- Prepare clinical and progress notes; complete documentation timely.
- Participate in agency‑sponsored in‑service programs; maintain or improve competency.
- Participate in the agency’s QAPI program.
- Supervise, teach and provide clinical direction to other nursing personnel; assign aides and supervise LPNs/LVNs and paraprofessionals; conduct competency evaluations if qualified with required experience.
- Provide effective communication to patients/clients, family, team, and health professionals.
- Perform on‑call responsibilities as assigned.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State license as a Registered Nurse or multi‑state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in an NLC state.
- Current Driver’s License with reliable transportation.
- One year of experience as a Registered Nurse in a clinical care setting; home health experience preferred.
- Proof of current CPR certification.
- Excellent knowledge of nursing care methods and procedures.
- Excellent knowledge of emergency care.
- In‑depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times.
- Team player with excellent communication and interpersonal skills.
- Responsible and compassionate.
- Outstanding organizational and multi‑tasking skills.
- Patient with excellent problem‑solving skills.
