Job Overview
The Nurse Navigator in Infusion coordinates and administers patient care treatment regimens, research components, and program outreach for patients diagnosed with cancer. The role supports patients and their families in navigating the array of services and programs offered by the hospital and community, providing personalized clinical and high‑touch care, and advocating for whole‑person health.
Responsibilities- Assist patients and families in managing services and programs to meet health and wellness needs.
- Provide personalized clinical and high‑touch care, advocating for integration of physical, mental, and spiritual health.
- Participate in health screenings, deliver patient education, and make appropriate referrals.
- Conduct direct clinical care and cultivate relationships with patients, families, physician partners, healthcare team members, and community partners.
- Administer and manage treatment regimens and collect data related to contacts and referrals as outlined by department manager.
- Consistently demonstrate behaviors that align with PSJH core values and the hospital’s mission.
- Bachelor’s Degree in Nursing.
- California Registered Nurse License.
- Oncology certification (OCN, AOCN, AOCNP, AOCNS, CBCN, or NCBC – Breast Patient Navigator) or successful completion of one of these certifications within 1 year of hire.
- Five (5) or more years of experience as a Registered Nurse in a healthcare setting.
- Three (3) or more years of experience in oncology nursing.
- Master’s Degree in Nursing (preferred).
