Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Alpha Omega Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

Schedule: Monday - Friday, 8 am - 4 pm

Territory: Carrollton, South Dallas

In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. • Respond to clinical referral information with care and expertise.
• Collaborate with the Clinical Director to assess eligibility and suitability of clients for hospice services.
• Coordinate care and provide comfort and support for patients, ensuring quality of life.
• Deliver high‑quality nursing care to ease pain and bring solace.
• Educate and advocate for patients and families, encouraging participation in personalized care plans.
• Participate in call duties to provide support when needed.

• Current, unencumbered Registered Nurse license.
• Minimum one year of RN experience in a medical‑surgical/acute care setting (hospice experience a plus).
• Computer proficiency and familiarity with EMR systems.
• Current CPR certification, if required by state.
• Valid driver’s license and reliable transportation.

$75,000 - $93,000 per year (average pay range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
