Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in patient homes or SNFs. You will review health history, medications, and risks to identify gaps in care and coordinate with PCPs.
Responsibilities include adult/geriatric assessments, ordering tests, collaborating with physicians and care teams, and documenting findings per Matrix guidelines. Travel up to 80–90% across the assigned state is required, with FT opportunities available.
