Summary

Under the direction of the department leader and supervision of primary nurses, this position will provide continuous observation and monitoring of patients at high risk of causing injury to self or others during their hospital stay. Safety performs a variety of patient care tasks. Population served by this group is all age groups.

Qualifications
  • American Heart Association (AHA) Basic Life Support (BLS) for healthcare providers
Preferred
  • Successfully complete the Non-Violent Crisis Intervention within six (6) months of hire or position entry.
Essential Functions
  • Has knowledge of and adheres to Carson Tahoe Health’s policies, procedures, and unit standards of care.
  • Attends 75% of scheduled staff meetings/unit conferences. Attends/completes 100% of all required Unit and Hospital education and Employee Health annual requirements by the established deadlines.
  • Maintains required license and certification requirements on provided platform.
  • Maintains competencies to reflect current skill level.
  • Monitors and improve safety for patients who remain a danger to themselves or others despite reorientation, redirection, and other alternatives to having a sitter.
  • Monitors patients using direct line of sight, either at the bedside or with the use of video monitoring equipment.
  • Collaborates with nursing staff to determine appropriate items needed in the patient room.
  • Monitors the care environment on a shift-by-shift basis, removing items deemed potentially harmful to patients.
  • Demonstrates customer-oriented approach by treating patients, staff and visitors with kindness and respect.
  • Reports any observations of increased agitation, confusion or change in condition to nursing staff.
  • Documents observations and care appropriately in the electronic health record.
  • Utilizes resources and materials in an efficient, safe, and cost-effective manner.
  • Redirects and intervenes to help prevent patient injury or fall.
  • Maintain confidentiality of patient information.
  • Maintains an orderly and clean environment for patients and co-workers.
  • Assists patients to the restroom and maintains direct line of site observation of patients while in the restroom.
  • Assists in turning or repositioning patients in bed or from bed to chair, utilizing appropriate assistive equipment when necessary.
  • Assists with limited activities of daily living
  • Setting patient up for a chair bath
  • Setting up and removing meal trays
  • Recording percentage of meals eaten
  • Setting patient up for oral care
  • Assisting with dressing.
  • Provides comfort needs of patient and/or family members.
  • Participates in therapeutic communication with patients.
  • Performs other related duties as assigned.
