Gentiva Hospice seeks an After hours RN Care Manager to provide high-quality hospice care outside standard hours. This role ensures seamless coverage, continuity of care, and strong clinical oversight for patients and families during nights, weekends, and on-call hours.
The nurse will perform assessments, manage plans of care, educate families, and collaborate with the IDG to coordinate care after hours and maintain compliance and documentation.
