As a valued Occupational Therapist at Center Well Home Health, you'll be instrumental in assisting patients to regain their strength, mobility, and independence right from their homes. By providing tailored rehabilitation care, you'll empower individuals to overcome challenges, effectively perform daily activities, and enhance their quality of life.

Your Role as a Home Health Occupational Therapist:

Evaluate patients' daily living and work skills, devising therapeutic retraining programs with clear and measurable goals.

Administer and assess diagnostic tests to inform physicians about patients’ functional capabilities.

Collaborate with the patient's physician and health care team to create or revise the Plan of Care.

Deliver therapeutic interventions and guidance to patients as per physician's instructions focused on improving strength and coordination.

Enhance and tailor retraining programs to align with individual patients’ behavioral goals while adhering to overall rehabilitation guidelines.

Work with vocational team members to develop and implement customized vocational plans suited to patients’ needs and abilities.

Regularly update and inform the physician, rehabilitation staff, and family on the patient’s progress and status.

Engage in care coordination and discharge planning, serving as a resource for the healthcare team.

Create community reintegration activities to assist patients in physical conditioning and psychological adjustments, collaborating with the interdisciplinary team.

Design and recommend specialized adaptive equipment to improve living or working conditions and enhance participation in rehabilitation and community activities.

Train and support family members and staff in executing retraining programs focused on daily living skills.

Supervise and evaluate the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.

Maintain precise documentation of patient care, interventions, and evaluations on the same day services are provided, including interim order documentation from physicians.

Required

Experience and Skills:

Degree from an accredited Occupational Therapy Program.
Minimum of six months of occupational therapy experience preferred.
Experience in Home Health is a plus.
Current and valid OT licensure.
Active CPR certification.
Strong organizational and communication skills.
Valid driver's license, auto insurance, and reliable transportation.

Compensation: Pay range of $49.00 - $69.00 per visit/unit, equating to an annual base pay of $77,200 - $106,200.
