Responsibilities- Execute a comprehensive social services program, offering counseling and support services to residents and families
- Assess and address residents’ medical-related social needs
- Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team
- Advocate for residents' rights while liaising between all parties involved
- Manage discharge planning efficiently
- Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights
- Part time hours (25 hours a week)

- Prior experience in long-term care is a plus not required
