Responsible for providing nursing services to the Hospice Services patients and as prescribed by the physician to be in compliance with the state's Nurse Practice Act, Federal Regulations and any other regulations defining Georgia's Hospice Aide services.
The Licensed Practical Nurse is responsible for the provision of a number of services to meet the patients' personal care needs and to promote their comfort and dignity.
ESSENTIAL DUTIES AND RESPONSIBILITES
- Provides direct patient care as defined in the LPN State Nurse Practice Act
- Implements current nursing practice following a patient assessment and an approved Plan of Care initiated by the Hospice Services Registered Nurse.
- Provides accurate and timely documentation of Hospice patient services to reflect the Plan of Care.
- Assesses and provides Hospice Services patient/family/caregiver education and information pertinent to diagnosis and Plan of Care.
- Participates in coordination of agency services, appropriately reporting the identified needs for agency Interdisciplinary Group members.
- Uses equipment and supplies effectively and efficiently.
- Provides appropriate pain/symptom management; evaluates Hospice Services patient's response(s) to treatments/medications.
- Participates in personal and professional growth and development. Also participates in the organization's quality management program.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to the work of committees, work groups, project management, and other collaborative efforts of the System.
- Performs other duties as necessary to ensure the success of the System.
- Ability to effectively communicate and present information in a clear and concise manner, both orally and in writing.
- Transports self to patient's home/Facility, may be in multiple locations in a workday
- Ability to establish and maintain effective working relationships with associates, department heads, administrators, member organization presidents and their leadership teams, and the general public.
- Nursing degree from an accredited college or university or is a graduate of an approved LPN program
- Current license / registration by state of hire as a LPN.
- Current CPR certification.
