Qualifications

Candidate must be a graduate of a state certified Nurse’s Aide training program, in good standing with the State Board of Nursing. Experience with death or dying patients is helpful. Candidate must embrace the hospice philosophy. A current driver’s license, auto insurance and a dependable means of transportation are required.

• Assist with personal care of patient, i.e., bathing, feeding, bed linen care, mouth care, vital signs, transferring and shaving.
• Document in patient’s chart.
• Follow infection control and safety measures.
• Assist with patient/family teaching concerning patient care and care for the caregiver.
• Bereavement activities as requested and/or able.
• Clean and monitor supplies.
• Attend and participate in staff and patient review meetings when able.
• Maintain and uphold the Hospice Code of Ethics.
• Work as a Hospice of the Premier care Hospice Team member.
• Keep Patient Care Coordinator/Nurse Case Manager informed of condition of patient/family.
• Maintain appropriate number of supplies at patient’s residence.
• Provide emotional support to patient/families.
• Reports directly to the Nurse Case Manager, and/or Clinical Manager.

Willowbrook Hospice will provide orientation and training, as necessary, to assist the CNA in performing the responsibilities listed above. Trainings may be in-house, or outside seminars or approved self-study. Documentation of orientation and training will become part of the personnel file.
