The Care Manager, as part of the interdisciplinary team, assesses, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post-hospitalization. This role involves assessing patients’ health needs, developing individualized care plans, and coordinating services to optimize health outcomes. Effective communication with patients, families, and healthcare providers is essential. Additionally, the Care Manager advocates for patients’ needs, promotes health education, and facilitates smooth transitions of care across healthcare settings.
Clinical expertise, critical‑thinking skills, and compassion are required to improve patient well‑being and healthcare outcomes. All duties are performed in alignment with the Mercy mission, values, and service standards.
- Graduate of an accredited School of Nursing
- Bachelor of Science in Nursing (Required)
- 1 year of acute‑care hospital setting
- Current RN License in the state of employment
- BLS (Basic Life Support) at hire date, or within 90 days
- Certification in Case Management
- 2 years acute‑care hospital setting
- Care Management or Utilization Management experience
