Overview
The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to the assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate the patient’s plan of care and maximize the ability to remain in a community setting. In collaboration with the physician, the nurse identifies at risk patient populations who require care management services.
The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary health care team, as well as community resources. The Ambulatory Care Nurse understands the needs of the organization and supports the mission, values, and management of patient care services. The ACN actively supports and incorporates ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, Wake Med’s Division of Nursing Professional Practice model, and Wake Med’s Patient & Family Centered Care (PFCC) model.
Registered Nurse required.
EducationDiploma Nursing or Associate’s Degree Nursing required.
