Glenaire is an accredited, Presbyterian-sponsored continuing care retirement community (CCRC) for seniors located in Cary, N.C. Cary is a unique city that has been ranked by Money Magazine as one of the Best Small Cities in America in which to live. Cary is located in the Research Triangle Park (RTP) area of North Carolina, close to Raleigh, Durham and Chapel Hill.
Glenaire offers a full continuum of care on the premises featuring all private accommodations in our Independent Living, Assisted Living and Skilled Nursing.
- Participates in collection of data from the resident or other appropriate source
- Recognizes the relationship of assessment findings as related to resident’s diagnosis
- Records and reports the results of the nursing assessment. Determines immediate need for intervention.
- Participates in identifying resident’s needs through suggestion of goas and interventions reviewed by RN.
- Participates in implementing the resident’s health plan of care
- Prioritize and perform interventions
- Recognize responses to interventions
- Modify immediate interventions based on changes in resident status
- Reports and records resident’s response to nursing interventions
- Suggests revisions to plan of care for review by RN
- Responsible for accurate and timely documentation
- Demonstrates effective communication skills with residents, representative of resident, physicians, and all members of the household team
- Documents in accordance with facility policies and procedures using the electronic medical record and all other communication tools
- Receives and transcribes physician orders according to policy
- Gives and receives a complete report at the beginning and end of assignment: includes correct counting and documentation of controlled substance
- Participates in teaching and counseling of residents and families
- Teaching/Counseling uses established teaching plans and follows established policies and protocols
- Participates in the Household to organize the work of the household and establish priorities.
- Assign nursing care activities to other LPNs
- Delegates resident care to verified Certified Nursing Assistants; retaining accountability for nursing care
- Graduate of an approved school of nursing
- Currently licensed as a LPN in good standing with the North Carolina Board of Nursing
- Current CPR Certification
- Previous experience with geriatric residents desirable
- Good physical and mental health
- Must be able to walk and stand for long periods of time
- Must be able to assist with lifting, turning, and positioning residents
- Must be able to push and manipulate equipment
- Views self as part of a team, not always the leader
- Identifies, promotes, and models person-directed care
- Views the household as the resident’s home and works to create attributes of home
- Understands the importance of consistency of caregivers for residents
- Facilitates residents and household team members in shared problem solving, decision making, and planning
- Participates in problem solving complex medical/psychosocial situations related to resident choice and risk
