Position: SOCIAL WORKER III

Department: Hertford County Department of Social Services
Location: Winton, NC
Salary: Starting at $41,699.84 annually

Position Summary

Hertford County Department of Social Services is seeking a qualified and motivated Social Worker III to provide case management services for the Special Assistance In-Home Program and In-Home Aide Services. This position plays a vital role in supporting elderly and disabled adults by helping them remain safely and independently in their homes while receiving appropriate services and assistance.

The Social Worker III is responsible for assessing client needs, developing service plans, coordinating care, and monitoring services to ensure client safety and well-being. This position also provides backup support for Adult Protective Services, Adult Day Care, Adult Home Specialist duties, and Child Protective Services when needed.

Essential Duties and Responsibilities

Case Management and Client Services

Conduct assessments to evaluate client needs, strengths, and eligibility for services

Develop and implement individualized service plans in collaboration with clients, families, and service providers

Coordinate services with healthcare providers, mental health professionals, and community agencies

Monitor services through regular client contact, home visits, and documentation

Maintain accurate and timely case records and reports

In-Home Aide Services

Manage a caseload of approximately 5–10 clients

Conduct annual reassessments and service plan updates

Monitor service delivery and ensure clients receive appropriate care

Protective Services and Program Support

Provide backup support for Adult Protective Services, including evaluating referrals and assisting with investigations

Provide backup support for Child Protective Services as needed, including participation in on-call duties

Assist with monitoring adult care facilities for compliance with state regulations

Participate in emergency response operations, including disaster shelter support when needed

Additional Responsibilities

Maintain compliance with federal, state, and local regulations and policies

Collaborate with internal departments and external partners

Perform other duties as assigned by the Supervisor or Director

Knowledge, Skills, and Abilities

Knowledge of social work principles, case management practices, and human services programs

Ability to assess client needs and develop effective service plans

Strong communication, documentation, and organizational skills

Ability to work independently and manage multiple priorities

Ability to work effectively with diverse populations and community partners

Knowledge of applicable state and federal regulations preferred

Minimum Qualifications

Education and Experience:
One of the following is required:

Master’s degree in Social Work from an accredited institution

Bachelor’s degree in Social Work and one year of related experience

Master’s degree in a human services field and one year of related experience

Bachelor’s degree in a human services field and two years of related experience

Bachelor’s degree in any field and three years of directly related human services experience

Additional Requirements:

Valid North Carolina driver’s license

Reliable personal vehicle available for work use

Ability to participate in on-call rotation and emergency response

Work Schedule

Monday through Friday, 8:30 AM – 5:00 PM
Participation in on-call rotation every 6–8 weeks and emergency response duties may be required.
