This position is located at Alexander Correctional Institution in Taylorsville, North Carolina. Alexander Correctional Institution is a closed custody male facility.
A Correctional Case Manager is responsible for helping manage an incarcerated individual’s rehabilitation, institutional adjustment, and reentry planning. This position reports to a Correctional Programs Supervisor.
Key Duties:- Conduct interviews and reviews to evaluate an individual’s criminal history, educational background, mental health needs, substance abuse issues, and risk factors.
- Create individualized case plans that outline goals, required programs, treatment services, and behavioral expectations.
- Track progress and update plans as needed.
- Refer individuals to educational, vocational, counseling, substance abuse treatment, and other rehabilitative programs.
- Coordinate with facility staff and external service providers.
- Keep accurate case records, progress notes, assessments, and reports.
- Prepare documentation for classification reviews, parole hearings, and other administrative processes.
- Assist with transition planning by helping individuals obtain identification, housing resources, employment assistance, community referrals, and other support services needed after release.
These duties support both institutional safety and the successful rehabilitation and reintegration of incarcerated individuals into the community.

Salary Grade Range: $37,377.00 - $65,408.00
